Tesla Model S Safety Concerns

Tesla Motors claims that their Model S car is one of the safest cars ever to be tested.  Despite receiving the highest safety rating from the NHTSA (National Highway Traffic Safety Administration)[1], some of the product cars have had potentially life-threatening design issues.  Recently, Tesla was forced to recall certain Model S vehicles manufactured between May 31, 2012 and November 12, 2015 when it was discovered that certain vehicles were equipped with driver or front passenger seat belts that may have been improperly connected[2].  The general use of lithium batteries in motor vehicles have resulted in significant fires which destroyed vehicles on two separate occasions[3].  Luckily no one was hurt.  In addition the most recent cause of concern for Tesla Model S, is the investigation of their autopilot software after a fatal auto accident involving the driver of a Tesla Model S and a tractor-trailer[4].
